To date, he is the only board-certified rhinoplasty surgeon who practices closed rhinoplasty exclusively. Your nose plays an important role in your overall attractiveness and the “personality or character” of your face. In addition, it usually reflects your ethnic heritage and links you to your family's roots and ancestry. However, few people are born with a perfectly shaped nose that is optimally balanced with the rest of their facial features.
And, if you're very unhappy with the way your nose fits your face, a lack of trust can permeate your entire life. You may feel withdrawn, anxious, or even depressed because of your nose. Rhinoplasty is perhaps one of the most challenging and sensitive surgeries we perform. In fact, it may be the most demanding and personally satisfying procedure that exists in the entire plastic surgery.
As you can imagine, rhinoplasty is a complex and challenging operation that should be left to the most qualified surgeons. Every time the central feature of the face changes, it will affect the rest of the face. Excellent technique and a detail-oriented approach are crucial to avoid undercorrection or overcorrection. This is a millimeter surgery and fewer and fewer surgeons are trying to specialize in this surgery.
Your surgeon must perform hundreds of cases a year to mark your experience with different types of noses. All noses are so different from each other that it is mandatory to see a surgeon who deals with anything that comes through the door. Ghavami learned from renowned rhinoplasty experts during his plastic surgery training at the University of Texas Southwestern Medical Center in Dallas. UTSW has a worldwide reputation for training rhinoplasty surgeons; in fact, it has hosted the Dallas Annual Rhinoplasty Symposium for more than 30 years.
Ghavami has been honored to be a member of the conference's faculty for nearly a decade, and he has shared what he has learned with other doctors both at the annual conference in Dallas and around the world. Ghavami also serves on the executive board of the exclusive Rhinoplasty Society, an elite organization dedicated to promoting the education, art, and science of this sought-after subspecialty of plastic surgery. He has been a professor at the American Society for Aesthetic Plastic Surgery (ASAPS) and has lectured around the world for the International Society for Aesthetic Plastic Surgery (ISAPS) and the Pan-Pacific Surgical Association (Hawaii Plastic Surgery Symposium), which he co-chaired. He is currently also co-chair of the symposium on rhinoplasty at the ASAPS annual meeting.
Ghavami is a widely published author of textbook chapters and articles on rhinoplasty. He has made numerous appearances on television programs broadcast nationally to talk about his advanced cosmetic surgery techniques. Ghavami honors the diversity and individuality of its patients. Design surgical plans that take into account the unique needs and objectives of each patient.
The “cookie cutter” approach is never used and shortcuts are never taken. As you browse our rhinoplasty photo gallery, you'll notice that you have noses that look very different from each other; this reflects the variety and uniqueness of each nose and the work done. Everyone's anatomy is completely different. The way in which skin, bone and cartilage interact creates a challenge, such as that of a unique musical composition. If these structures are properly addressed, the results can come together as a beautiful symphony.
While some patients will want to have “designer” nose surgery, Dr. Ghavami believes that no one should be able to notice that his patients have had a nose job. It strives to create possible natural-looking outcomes that fit each patient's unique personality and goals. This requires the use of meticulous techniques that he has perfected over his many years of practice and on which he has lectured for nearly a decade.
Ghavami evaluates all potential candidates during a consultation at his office. Suitable candidates are generally people who are at least 15 years old and whose nose has stopped growing. Candidates must be in good physical and psychological health and must not smoke. They must demonstrate that they seek rhinoplasty of their own free will (not to satisfy anyone else) and that they have reasonable goals and expectations regarding the surgery. Ghavami uses one of two rhinoplasty techniques.
Closed rhinoplasty is performed through incisions inside the nostrils. Open rhinoplasty requires making an additional incision in the strip of skin that separates the nostrils (called a columella).). Ghavami gently separates and lifts the skin “envelope” of the underlying nasal tissue and structures for a detailed three-dimensional view. We believe that open rhinoplasty is often necessary to offer a truly demanding approach and to minimize revisions.
Open rhinoplasty is like opening the hood of a complicated engine, such as that of a Lamborghini, to fix it, while a “closed” rhinoplasty (or what some unfortunately call “scarless”) is like trying to fix that engine in an expert way with the hood down and going through the grill with a flashlight. Ghavami follows a predetermined treatment plan, which may involve adding, removing or otherwise modifying tissue, cartilage and bone to give them the desired shape. Ghavami can remove cartilage from the septum, ear, or ribs and add it to the nose (this is common in cases of ethnic or revision rhinoplasty). Ghavami can correct the deviation of the nasal septum and enlarged turbinates or make other improvements to the structures of the nose to improve respiratory function. When you're done, cover your nose with skin again and close the incisions with stitches.
Patients may have varying degrees of swelling, bruising, bleeding, congestion, nose pain and pain deaf head for several days after surgery. These side effects are temporary and go away quickly. Most patients take about a week off from work or school. Contact lenses can be worn immediately after surgery, but they may need to be taped to the forehead or placed on the cheeks for up to four weeks.
Ghavami can demonstrate bandaging and massage techniques to improve results. As the swelling decreases, the results of the operation become visible. Swelling may reappear from time to time during the first year after the procedure (usually more noticeable in the morning). Ghavami encourages patients to wait a year before judging the final results of rhinoplasty.
However, most of the general shape of the new nose can be seen and judged around of the three months. It is very important for the doctor to understand your vision for the desired nose. Together during the consultation, the two of you will spend time discussing your ideal results. They will look at images of your current nose, as well as images of the nasal shapes you like, and they will decide how to improve it to better complement your facial balance.
Ghavami will also work with you to ensure that your nose matches your ethnic heritage. The final results depend on the bone and cartilage structure of the nose, facial shape, skin thickness and age. Chapters and articles from Ghavami's specialized textbooks are shown in our Publications section. Ghavami has also appeared in the national media to talk about his advanced cosmetic surgery techniques.
AFRICAN AMERICAN RHINOPLASTY IN WHICH THE NOSE WAS COMPLETELY LENGTHENED AND REMODELED, ALONG WITH THE NARROWING, NARROWING, AND REMODELING OF THE NOSTRILS. THE TIP WAS ALSO REFINED, PRODUCING THIS SPECTACULAR, LIFE-CHANGING RESULT. Ghavami's results make it possible to cover up the preoperative view and only look at the postoperative image, without knowing that it was a nose that had been surgically modified. Ghavami is in high demand both by patients and by professionals and colleagues to share their knowledge and advanced surgical techniques.
He is regularly invited to give lectures, contribute to educational textbooks and help with scientific articles related to cosmetic surgery.
